Firstly, I do not agree with Crusader's form because he want's info very detail. Secondly, player must to have right to be private.
So this is my example of appliance:
1. Game Info: Name-in-game, USGN, activity, skills-in-game.
2. Personal Info: Real name, country, language, hobbies.
3. Reasons to be mod
